What Else You Should Know…
- Plans cannot be added to additional insurance plans or discounts.
- Payment for any treatment not included in the membership plan is due at the time of service. A 15% discount will be applied if paying with cash or check. A 10% discount will be applied if paying by credit/debit card.
- Plan payments are non-refundable. Plan is based on 12 revolving months.
- Plan is subject to change yearly.
- The membership plan does not cover procedures completed by a specialist outside of our practice.
- Plans and copays cannot be paid by Care Credit.
